Question: Hello doctor

Exposure was on 30/8 /18 vaginal fluid on tip of penis after fingering in gap of 8-10 sec

Hiv duo test at 27 day negative of mine
Cbc on 23 day wbc 9000
Neutrophil 6900
Lymphocytes 1800

Cbc on 19 day wbc 7800
Neutrophils 5800
Lymp 1600


Even the girl tested negative on duo test after on 7 day of my exposure with normal cbc

3 day before my exposure with my friend she had sex with guy unprotected later he tested positive after 1 week


Here is my question
1) will wbc go up or down in acute hiv
2) do i need to worry for hiv
3) protected sex only finger transmission of fluid

Fingering carries a negligible risk of HIV transmission as per CDC, XXXXXXX USA. Even the act contaminated your penis with vaginal fluid the risk, the HIV risk is still negligible only as long as there was no abrasion either in your finger or penile skin.
CBC values would not be of much help in diagnosing or valuing the prognosis. It would support other findings, that is all.
Your non reactive HIV DUO test 27 days ruled out HIV 1 infection. DUO test consists of two components:
1.Detection of P24 antigen which would detect the HIV 1 infection as early as 7 days and became conclusive by 3-4 weeks.
2. Detection of antibodies against HIV 1 and 2, which will become sensitive after 6-8 (window period) weeks and become conclusive by 12 weeks. This test is highly reliable to the extent of 99% after 12 weeks.
Throughout the world except in certain XXXXXXX countries the incidence of HIV 2 is less than 1%.
In XXXXXXX it is less than 0.5%. So nothing to worry. You are almost free from HIV infection. Nothing to worry.

Welcome back.
To say frankly, I am yet to see anybody with acute infection in HIV disease. Almost all my cases seen during my 33 years of experience with HIV infection were either diagnosed or referred to me with advanced stage along with symptoms and few cases were spouses of the HIV positive individuals without any symptoms and definitely not with acute infection. As per text books also Acute HIV infection can be diagnosed with molecular HIV tests only and not with either CBS or total WBC count or Neutrophill count.
In Other HIV patients WBC total count varies widely according to symptoms or associated infection.

True, with many instances. Not always. Moreover this low WBC count can be taken as a supportive finding and not an ultimate one.
Total WBC count has a wide normal range from 4000-11000 cells per ml. So the lowest WBC count within this range can happen in many situations and cannot be taken as a normal. Only when the count went below 4000 has significance.

Means wbc wont rise in acute infection if it rise means body has antibodies we can say and lymphocytes will also increase with production of antibody

My wbc was 7800 on day 19 and wbc on day 23 was 9000
