I've Been Having Some Annoying Symptoms Over The Last 10

Question: I've been having some annoying symptoms over the last 10 years now. I'll try to sum my symptoms up below. Started 10 years ago few month after I met my gf. Been having symptoms ever since. My biggest issues is that after peeing I can't empty the urethra which results in dripping as soon as I sit or do a movement that presses the area. This started together with my other symptoms. Pain after ejaculation, not instantly but starts shortly after and will continue for the rest of the day. Also if I get an erections within the same day it's causing even more symptoms and hurts. Standing will only make this worse and the pain is between the scrotum and anus (perineum?) and resembles a heavy feeling ache. If I empty my bladder after ejaculation I won't get the same pain and can often feel all fine, but having another ejaculation on the same day will still causing issues. I also experience pain sometimes while peeing, not very often and sometimes small amounts of pee which usually results in a burning sensation. Also I feel my stream got weaker when this started 10 years ago. I thought all this was caused by a varicocele I have too but I've learned that these symptoms are not of varicocele. That made me research into this about possible prostatitis. I've seen my doctors several times and thought the varicocele was the issue and just lived with it, but since reading of this condition I believe my symptoms sounds a lot like chronic prostatitis? I struggle with bowel issues which I've been informed after many tests are ibs and I have a general problem with anterior pelvic tilt, poor back posture and a very high sedentary lifestyle for most of my life. I'm normal weight and 29 years old. Also I've had UTI once around 5-6 years ago which also makes me raise an eye on this condition, given its more uncommon for a male to get uti. Please help me understand if these symptoms sounds familiar or if you don't think it sounds like chronic prostatitis.

Brief Answer:
You mostly have Chronic Prostatitis .

Detailed Answer:
Hello
Thanks for query and highlighting your problems in detail which you have since last 10 years .

The symptoms like dribbling after urination ,pain in perineal region,
painful urination ,and pain while ejaculating is mostly due to chronic infection of Prostate (Chronic Prostatitis ).

You need to consult qualified Urologist for clinical and Digital Rectal Examination (DRE) and get following basic tests done to establish the diagnosis .
1) Urine routine and urine culture
2) Semen examination and Semen culture
3) Ultrasound Scanning of abdomen and pelvis .
4) Culture of a Urethral discharge obtained after prostate massage .
5) Endoscopic examination of urethra and bladder (Cystoscopy)

Please note that the symptoms that you have are typical of Chronic Prostatitis and in many cases result of these test are not conclusive are normal but has to be carried out to rule out other pathological conditions .
In the mean time you can start taking antibiotic like Bactrin D S and Nitrofurantoin with anti inflammatory drug like Diclofenac twice daily for two weeks .

Doing gental Prostate massage everyday does helps to relieve congestion of Prostate and gives very satisfactory results,

Further treatment will depend upon the results of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Thank you for your answer.

Is chronic pelvic pain syndrome and pelvic floor dysfunction the same thing or could both cause the symptoms that I am experiencing?

I've been tested many times and there's no bacteria in my pee also I've been through 2 months of Tetracycline for acne which should also work on urinary infections as far as I'm informed.
How likely is it that my symptoms are caused by bacteria when they've been with me for 10 years + up to now and mainly come if I have an ejaculation or sit down a lot over a period?

I recently started having problems with my bowels and went to get checked for all kind of things but they never found anything and labeled it as IBS, could a pelvic floor dysfunction be a cause for all of my issues?

My biggest issues with the IBS symptoms are that I feel often like I can't fully empty out my bowel movement, like I have to strain or that my muscles don't have the same 'push' power that it used to have and then I continue feeling a need to go to the toilet but can't go and that causes a lot of irritation for me.

I've been checked for problems with my urinary issues many times but they never found anything and my pain usually is between scrotum and anus where it lingers for hours.

Do you know why peeing after ejaculation can minimize the pain?

Brief Answer:
Chronic Pelvic Pain syndrome and Pelvic floor dysfunction both are same

Detailed Answer:
Hello
Thanks for follow up .
Chronic Pelvic Pain syndrome and Pelvic floor dysfunction are one and the same entities and also called Chronic Non Bacterial Prostatitis .

Chronic Non Bacterial Prostatitis is a condition where the patient have all the symptoms that you have but there are no positive findings detected on relevant laboratory investigations.

It is the most notorious disease where patient and treating Dr both are handicapped (All the tests are being reported to be normal).

As regards IBS it is said to be due to Neuro Muscular imbalance in large bowel resulting into irregular bowel function of absorption of water and motility of large bowel giving rise to symptoms like tenesmus, loose motion or constipation and pain and discomfort in lower abdomen .The main cause for the same is a stress factor .It is also never cured completely and have relapses off and on .

The drug like Mebverine twice daily does help to regularize bowel function and gives relief to certain extent .

One has to modify diet habits like avoiding fibers in diet ,salads, milk products ,Oates, raw and junk foods.

Thanks and Regards
