The possibilities I can think of with your short history are as follows:
Infection in one of the ducts.
Inflamed vein.
Inflamed arolar gland.
Cancers per say are painless hence least possible but needs a consideration as you are 36 years old.
Please give additional information for me to assist you better:
Is the skin red over it?
What is the size?
Marriage? Children? Breast feeding? OC pills?
I know it is difficult- if possible post clear and focused pictures?
Is the cord extended to outside of areola?
Any other information please.

Thanks for the picture and feedback.
The picture shows one enlarged Areolar gland or Gland of XXXXXXX and also a blue line indicating possibly inflamed vein. If the vein is inflamed this is usually due to a condition called idiopathic superficial thrombophlebitis.
This in turn can give pain in the armpit due to inflamed lymph node and/or similar inflamed vein.
It is worthwhile to get a proper clinical examination, mammography, ultrasonography and FNAC if there is any lump within the breast. Suspicion of anything in the breast should be ruled out.
