Question: sometimes I see black and white forms in front of my eyes. these forms last about 5 or so minutes and then they go away but then I am left with a dull headach. I used to get this problem but it has become more frequent

Based on your description , you are likely describing visual aura that precedes a migraine headache. However, an ophthalmologist must be consulted to rule out any other cause. Fundoscopy and slit lamp exam should be done to look for and rule out possible retinal tears or detachment. Clumps of unresolved vitreous gel can also cause visual floaters.

If the eye exam is normal then treatment for the headache can include initial abortive therapy with NSAIDs. Prophylaxis with beta blockers or calcium channel blockers can be advised if the symptoms recur frequently.
