Question: On 14/2/18 I had master health chek up one of the serum result showed PSA 19.79, As advised consulted urologist who did DRE and prescribed Urimax D one in the night and zanocin 200 mg 101 and did plan for biopsy. I took second opinion and the Urologist did DRE and found Nodular prosate L bon firm PR Gr.I and prescribed urimax 0.04 for one month and niftas 100 mg for 15 days and advised to come with PSA Urine routine and culture test which showed PSA 20.10 urine P vulgaris and prescribed Ceftum 500 mg twice for 20 days and 250 mg twice for 10 days and urimax for 6 months. Now PSA level shows 13.48. When I was taking nifas 200 mg for 15 days and urimax for one month the psa came down to 12.1 and now after taking ceftum the psa level slightly increased. Shoud I to presume that should again take niftas 100 mg and urimax 0,4 daily as this medicines had worked to bring down my PSA level Sir. Kindly advice and help me. My prostate volume 31 cc . Please help me sir

I have gone through the details. It looks like you are having Chronic bacterial prostatitis. Has the antibiotic course with ceftum over? When was the last antibiotic you took. Did you have any urinary complaints before you got your health check-up done?

Dear Dr. Following is my answer to your question:
Q1)Has the antibiotic course with ceftum over?
Ans:My antibiotic course with ceftum is over on 20/04/2018 and on 21/04/2018 I had taken serum test which showed the value of 13.48 which has risen from 12.10 on 21/03/2018 when the urologist has prescribed ceftum regiments on seeing urine culture test showed proteus vulgaris,
As regards second and third query I wish to clarify that just before 4 days as on 19/02/18 I was passing urine four to five times at night further I being a diabetes has not undergone lupid profile for more than 6 months therefore I wanted to have master health check from the Tamilnadu Government scheme. I did not take any medicines at that time for this urine problem. The PSA 19.79 was found only out of master health check and before that this test i did not under go. I had also undergone ultra sound under this scheme which showed prostate appears enlarged in size volume 31 cc and shows clcification prostatomegally grade IIi with prostatic calcification bladder thickness measuring 3.2 MM non distended urine residual volume 73 cc Sir.

Hi,

It is not advisable to take tab. niftas for a long time as it can cause some serious side effects. Chronic prostatitis is not easy to be treated. You have to follow up with your urologist and he may continue the antibiotic course for some more days. As you are asymptomatic there is nothing much to worry, but it is best if you go for a biopsy and rule out other causes. If you are having symptoms and still not responding to medical treatment, surgery is the last option.

Now I do not have any symptom. culture test also showing nil now. However since you are advising to see urologist for continuing antibiotics why not you advise me to continue ceftum 250 mg for some more days sir say for one month and then I shall go to urologist with psa report for continuing treatment sir. Kindly help me as i do not have symptom but psa only is my problem

Sorry for the delay in the reply.

As your culture report is nil, I would not suggest you take antibiotics as it will have no effect and unnecessary use of antibiotics may lead to resistance of the antibiotic. I would suggest you try natural methods for 1 month such as supplements such as green tea, probiotics like Sporolac, cranberry juice will help. Dietary changes, and lifestyle remedies like sitz baths. Physiotherapy may also help.

Meanwhile, you can continue urimax and take Tab. Immunace Forte once daily after food for 30 days. This supplement is rich in antioxidants.
