What Could Be Possible Reason For Intermittent Right Scrotal Pain?

Question: Around three months ago, I visited a urologist for intermittent right scrotal pain. The pain is dull, and feels like it comes from the back of my right testicle.

Urologist performed manual exam and felt that both testicles felt normal. Ultrasound was performed in December 2023 and was normal as to the testicles. However, two findings were reported related to the epididymides: (LEFT) Left epididymis: Normal size and vascularity. 9 mm echogenic focus in the left epididymal head, possibly a small lipomatous lesion. Left hydrocele: None. (RIGHT) Right epididymis: Normal aside from a 5 mm cyst with rim calcification in the body. Right hydrocele: None. Right varicocele: None. (<.2.5mm)

Ultrasound was repeated last week.

Testes were normal. These are the findings as to epididymides:

Epididymal heads:
Right measures 1.2 x 1.1 x 1.1 cm. No discrete abnormality evident.

Probable appendix testis noted measuring 7 x 5 x 3 mm. This was evident on 2016 examination is unchanged.

On the December 26, 2023 exam, a the small nodular echogenic was present within the body habitus. This region is not well visualized.

Left epididymal head measures 1.3 x 0.9 x 1.1 cm and contains a 6 × 5 × 4 mm nodular echogenic nonshadowing area. On the prior/December 26, 2023 exam this area measured 5 x 4 x 9 mm. This was not evident 2016. Appearance is nonspecific. Fat-containing lesion is a possibility. Some small granuloma could give this appearance. Complex spermatocele is also consideration. Epidermoid cyst less likely. More aggressive pathology less likely but difficult to exclude. Follow-up testicular ultrasound in 3-6 months suggested.

I have follow up with urologist in a few days to review the results. I am concerned about some sort of epididymal cancer and am frustrated that the area containing the cyst on the right side was not visualized.

1) Based on the measurements, it looks like the LEFT lesion got slightly smaller. Is this generally a good sign?

2) As to the right sided cyst that wasn’t visualized on the April ultrasound, is a calcified cyst in the body of the epididymis (as opposed to the head) still considered a benign finding?

3) Anything else based on these results that might put my mind at ease?

Based on the facts and reports that you have posted you seem to have a small lesion on left side persistently of the same size which is most likely to be a benign lesion

As regards a small cyst in art Epididymis it is a benign spermatic cyst and needs no treatment.

Do not worry about having cancer
